Applying
There are two ways to apply:
- Filling in a paper form
- Editing the Word document of the form to fill it in
Using the Paper Self-Nomination Form
If you don't have a copy of the self-nomination form, you can download the form from this link as a PDF file:

You can print this file out and then, after filling it in, turn in the form at the next CBAC meeting or mail it to:
